In-Home Support Services are designed to support individuals with developmental disabilities who reside in their own homes or family homes. The service focuses on building adaptive, social, and daily living skills, and ensuring the individual's health, safety, and well-being. Supports are customized to the individual's person-centered plan (ISP) and emphasize community integration, independence, and quality of life.

Our team will coordinate care amongst current providers, including the individual’s Support Coordinator or Case Manager to schedule initial screening, assessments, and interim/planning meetings.
Our clinical team develops and introduces a plan for supports which outlines and describes the specific supports to be provided based on the wants, needs and preferences of the individual.
We will work closely with the client and families to identify a schedule and routine. Upon approval of services, we will continue to coordinate care amongst current providers and implement supports based on the individual‘s plan for supports.
