IN-HOME SUPPORT SERVICES

In-Home Support Services are designed to support individuals with developmental disabilities who reside in their own homes or family homes. The service focuses on building adaptive, social, and daily living skills, and ensuring the individual's health, safety, and well-being. Supports are customized to the individual's person-centered plan (ISP) and emphasize community integration, independence, and quality of life.


Service Goals

  • Enhance individual independence and quality of life
     
  • Promote skill-building in self-care, communication, socialization, and community participation
     
  • Provide individualized supports to ensure health, safety, and emotional well-being
     

Description of Supports Provided

Supports may include, but are not limited to:


  • Assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
     
  • Skill-building to improve self-help, social, and adaptive skills
     
  • Safety and behavioral supports to address challenging behaviors
     
  • Community integration supports (transportation, shopping, recreation)
     
  • Medication assistance and health monitoring
     
  • Assistance with accessing and using community resources
     
  • Emergency preparedness and safety training


Characteristics and Needs of Individuals Served

  • Individuals with a diagnosed developmental disability
     
  • Individuals assessed and determined eligible for DD waiver services (FIS or CL)
     
  • Require support to remain safely and independently in the home/community
     
  • Range of functioning levels from mild to profound developmental disabilities

OUR PROCESS

Initial Contact and Screening

Our team will coordinate care amongst current providers, including the individual’s Support Coordinator or Case Manager to schedule initial screening, assessments, and interim/planning meetings.

Plan for Supports

Our clinical team develops and introduces a plan for supports which outlines and describes the specific supports to be provided based on the wants, needs and preferences of the individual. 

Authorization and Service Initiation

We will work closely with the client and families to identify a schedule and routine. Upon approval of services, we will continue to coordinate care amongst current providers and implement supports based on the individual‘s plan for supports.
