In-Home Support Services are designed to support individuals with developmental disabilities who reside in their own homes or family homes. The service focuses on building adaptive, social, and daily living skills, and ensuring the individual's health, safety, and well-being. Supports are customized to the individual's person-centered plan (ISP) and emphasize community integration, independence, and quality of life.

Description of Supports
- Assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
- Skill-building to improve self-help, social, and adaptive skills
- Safety and behavioral supports to address challenging behaviors
- Community integration supports (transportation, shopping, recreation)
- Medication assistance and health monitoring
- Assistance with accessing and using community resources
- Emergency preparedness and safety training
Who We Serve
PHS of VA provides services to individuals who may benefit from in-home and community-based supports, including:
