IN-HOME SUPPORT SERVICES

In-Home Support Services are designed to support individuals with developmental disabilities who reside in their own homes or family homes. The service focuses on building adaptive, social, and daily living skills, and ensuring the individual's health, safety, and well-being. Supports are customized to the individual's person-centered plan (ISP) and emphasize community integration, independence, and quality of life.


Service Goals

  • Enhance individual independence and quality of life
     
  • Promote skill-building in self-care, communication, socialization, and community participation
     
  • Provide individualized supports to ensure health, safety, and emotional well-being


 

Description of Supports Provided

Supports may include, but are not limited to:


  • Assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
     
  • Skill-building to improve self-help, social, and adaptive skills
     
  • Safety and behavioral supports to address challenging behaviors
     
  • Community integration supports (transportation, shopping, recreation)
     
  • Medication assistance and health monitoring
     
  • Assistance with accessing and using community resources
     
  • Emergency preparedness and safety training



Characteristics and Needs of Individuals Served

  • Individuals with a diagnosed developmental disability
     
  • Individuals assessed and determined eligible for DD waiver services (FIS or CL)
     
  • Require support to remain safely and independently in the home/community
     
  • Range of functioning levels from mild to profound developmental disabilities

OUR ADMISSION PROCESS

Initial Contact and Screening

Our team will coordinate care amongst current providers, including the individual’s Support Coordinator or Case Manager to schedule initial screening, assessments, and service plan meetings.

Support Planning

Our clinical team develops and introduces a plan for supports which outlines and describes the specific supports to be provided based on the wants, needs and preferences of the individual. 

Authorization and Service Initiation

We will work closely with the client and families to identify a schedule and routine. Upon approval of services, we will continue to coordinate care amongst current providers and implement supports based on the individual‘s plan for supports.

ROUTINE SUPPORTS

Adaptive Equipment / DME Support

  • Assistance with the proper use and maintenance of adaptive devices or durable medical equipment (DME).
     
  • Ensuring equipment is in good working order and used safely to promote independence and mobility.

Communication Support

  • Supporting verbal and non-verbal communication methods, including the use of assistive devices, visual supports, or communication boards.
     
  • Encouraging self-expression and active participation in conversations and daily decisions.

Dressing Support

  • Helping individuals select appropriate clothing for weather and activities.
     
  • Providing physical or verbal assistance with dressing while encouraging independence.

Restroom Support

  • Assistance with toileting needs, hygiene, and clothing adjustments as required.
     
  • Promoting privacy, dignity, and independence during restroom use.

Bathing/Showering

  • Assistance with showering, bathing, or sponge baths while maintaining privacy and dignity.
     
  • Monitoring water temperature and safety during hygiene routines.

Positioning / Transferring Assistance

  • Supporting individuals with safe transfers from bed to chair, wheelchair, or vehicle.
     
  • Implementing positioning plans to promote comfort and prevent pressure injuries.

Personal Appearance Support

  • Assistance with grooming tasks such as hair care, shaving, nail care, and overall hygiene.
     
  • Encouraging self-care routines that build confidence and self-esteem.

Medication Use and Administration

  • Reminding or assisting with medication administration per the individual’s plan of care and within regulatory guidelines.
     
  • Monitoring for side effects or changes in health status and reporting concerns promptly.

Housekeeping Assistance

  • Assisting with maintaining a clean, safe, and organized home environment.
     
  • Supporting daily chores such as vacuuming, dusting, and tidying personal spaces.

Meal Planning and Preparation

  • Assisting with meal planning that aligns with dietary needs and preferences.
     
  • Supporting cooking, portioning, and safe food handling to promote independence.

Transportation

  • Providing or arranging safe transportation to work, appointments, and community activities.
     
  • Supporting individuals in learning safe travel skills when possible.

Crisis Planning and Intervention

  • Recognizing early warning signs of distress or crisis and following individualized crisis prevention plans.
     
  • Implementing de-escalation techniques and contacting emergency supports when necessary.
